Transcript Evaluation Request Form (PDF)
Request an evaluation of prior coursework completed at another college or university. Evaluating previous college credits is optional unless you are planning to receive financial aid or Veterans Administration benefits. Transcripts will be evaluated after:
- A completed Transcript Evaluation Request FormĀ (PDF) is submitted to the Admissions and Records Office.
- All transcripts listed on the form have been received by the Admissions and Records Office.
- Student is currently enrolled (registered and paid) in at least one class within the UH system.
- Leeward Community College is the students declared home institution.
Once the evaluation is complete, students can view their evaluation on their MyUH portal account.
- The form is fillable using the AdobeĀ® Acrobat Reader. Complete this form and turn it in to LCC Admissions & Records Office
